Specifically, the CFT Specialist is responsible for facilitating time limited, outcome-focused Child and Family Team (CFT) Meetings with families, extended family, children/youth, caregivers, community representatives, service providers, and agency staff for all placement-related decisions in partnership with Child Protective Services (CPS). The CFT Specialist guides meeting participants through discussion of concerns, identification of strengths, and development of ideas to reach consensus regarding the least restrictive, best possible placement-related decisions. The CFT Specialist is required to complete session notes. The CFT Specialist works in a primarily office-based setting, under the guidance of a Clinical Program Manager.
JOB SUMMARY Works within the vision, mission, and philosophy of the agency and the Department of Child, Family, and Adult Services (DCFAS). Demonstrates advanced skills and abilities regarding facilitation, conflict management, and mediation of teams. Trains and provides leadership to peers in the implementation of Safety Organized Practice (SOP) in their engagement with families, supports, and system partners. Provides advanced cross-functional team facilitation to support and enhance the efforts of youth, their families/caregivers, and the system workers who are part of the Child and Family Team. Provides leadership, facilitation, engagement, and quality documentation for services planned and provided. The CFT Specialist works in a primarily office-based setting, under the guidance of a Clinical Program Manager (CPM).
Works collaboratively with others within the program and agency, as well as with system and community partners.
RESPONSIBILITIES AND DUTIES Demonstrates mastery of the CFT Specialist I position.Understands and demonstrates proficiency with interactions of Child Protective Services, Probation, Mental Health Providers, Educational System, and other complex systems in the context of a family's life, service delivery structures, and the community's resources.Supports in training CFT Specialist I in facilitating SOP Child and Family Team Meetings (CFTMs) and Emancipation Conferences (ECs).Explores, identifies, and incorporates community resources that may meet the needs of families.Demonstrates excellence in role modeling and partnership for others of how to partner with social workers, system partners, and service providers participating in child and family team meetings.Role models for other staff on how to engage youth, parents, and caregivers in different emotional states of well-being and individuals with special needs in order to ensure family voice, choice, and access.Participates effectively in internal and external multidisciplinary teams. Acquires, monitors, and evaluates information to provide effective feedback to CFT Specialists and CPM.Provides coaching and mentoring to others in the agency principles, concepts of Safety Organized Practice and reviews process of ROI with Social Workers, behavioral health, attorneys, families, and other team members.Supports peers, social workers, parents, children, and natural supports to identify, learn and access self-care strategies.Assumes leadership and increases participation of others in work groups, committees, and team meetings.Takes initiative to identify and resolve conflicts and recognizes strength in the diversity of families and teams.Initiates, organizes, and facilitates projects and activities as well as models effective prioritization of work tasks.Required to meet timeliness and documentation requirements.Required to facilitate CFTMs and ECs.Required to observe CFT Specialist I facilitate CFTMs and ECs, frequency determined by consultation with CPM.OTHER DUTIES AND RESPONSIBILITIES Practices self-care, remains aware that others may be contending with stress, and treats others with grace.Travels to facilitate.Travels to provide observation and live coaching to CFT Specialists.Supports CPM in facilitation of trainings and onboarding of new staff.Physical interactions with children of variable ages such as but not limited to games, sports, and other activities.Assists in monitoring data tracking of each CFT Specialist I.Performs other responsibilities, as assigned, to support department and business needs.JOB SPECIFIC COMPETENCIES Documents clearly and accurately.Encouraging customer focus.Leads team performance.QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. MINIMUM EDUCATION AND/OR EXPERIENCE Three (3) years experience working with youth/families with complex needs required. Experience with child welfare, juvenile probation systems, and/or mental health systems required.
PREFERRED QUALIFICATIONS Bachelor's Degree (BA/BS) in Behavioral Science, Education, or related field preferred.
Three (3) years experience in Healthcare/Medical – Psychology/Behavioral/Mental Health preferred.
HIGHLY DESIRABLE QUALIFICATIONS Speak, read, and/or write another language fluently.
Experience with Child and Family Team Meetings.
Experience and Knowledge of Sacramento County Child Protective Services.
