Job Description - Clinical Pharmacy Specialist (3312657) ResponsibilitiesReview and assess the quality of pharmacotherapy for patients in assigned coverage areas.Optimize drug therapy by identifying, resolving, and preventing drug-related problems.Process medication orders entered by prescribers and provide accurate information for safe medication use.Work independently within established pharmacy practice and departmental policies as part of the patient care team.Oversee the preparation, compounding, and dispensing of medications in accordance with regulatory agencies.Provide drug information and pharmacotherapeutic consultations to healthcare professionals.Monitor drug therapy to prevent contraindications, duplicate therapy, and drug interactions.Attend and participate in medical and multi-disciplinary rounds.Document clinical interventions and adverse drug events.Provide training/in-service education to Pharmacy staff.Participate in developing drug use educational materials and monitoring initiatives.Engage in cost avoidance and cost savings activities through drug utilization projects.Contribute annually to the Pharmacy Department newsletter.Assist the Clinical Pharmacy Manager with clinical clerkship students and pharmacy externs.Ensure proper drug security and control throughout the day.Maintain necessary record keeping and complete assigned medication inventory assignments.Perform additional duties as assigned by Pharmacy Director or Assistant Director.Minimum RequirementsAdvanced degree or Doctorate of Pharmacy from an accredited college, or equivalent years of advanced clinical pharmacy practice experience.Licensed to practice in the Commonwealth of Massachusetts.Excellent oral and interpersonal communication skills.Understanding of Pharmacy Department operations and relevant technology.Ability to prioritize and organize work under stress and time constraints.Knowledge of pharmacotherapy for safe and effective medication distribution.Knowledge of growth and development principles across the lifespan.Leadership abilities in Clinical Pharmacy Practice.Brigham and Women's Faulkner Hospital is an Affirmative Action Employer.
