Comprehensive Care Acp

Be an integral part of the team. The Comprehensive Care ACP is responsible for seeing the highest priority members either in-center or through the telehealth platform. In addition this position holds responsiblity for center metrics, coding, documentation, training, and education of other providers.
Qualifications & Requirements
• ANCC, AANP or PA Certification in Family, Geriatrics, or Adult (Gerontology PC or AC)
• Active Oklahoma NP or PA license or ability to obtain
• 1+ years of clinical experience
• Experience with EMRs, Advanced knowledge of ICD-10 diagnostic codes
• Medicare Advantage knowledge of HCC model and STARs measures
• Experience in gerontology, cardiology, internal medicine or endocrinology preferred
• Comfortable traveling within the market to multiple centers
• Willing/enthusiastic about working with Medicare-aged, medically underserved patient population
