Transportation of residents to and from scheduled appointments in a timely and safe manner.
Assure the safety of each resident when transporting to and from the doctor's office and/or hospital. Accompany ach resident on all medical visits. Stay with resident(s) until visit is complete. Observe the following guidelines at each visit: Sign in resident upon entrance. Present resident's documents to office personnel. Before leaving doctor's office, please observe the following: Obtain resident's documents. Obtain any new prescriptions. Ensure doctor has written on the doctor's order form. If follow-up appointment necessary, obtain appointment cards. After returning to facility, report to RCM and be prepared to answer questions concerning visit. Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle. Possess a valid state driver's license. Clean driving record. Clean criminal record. Long term care experience preferred.
Verbal ability is required to communicate with residents and to understand instructions received from nursing staff.
Ability to read and write and accurately report.
