Schedule: Monday- Friday; 8 hours/day
Position Overview: The Family Team Facilitator collaborates with youth, families, and informal and formal support to identify individualized needs for youth dually involved in the Juvenile Justice systems and develop a plan of care to meet those needs.
Major areas of responsibility for this job: Facilitating meetings with identified parties utilizing a wraparound approachCompleting strength-based assessments of all life domains, identifying both strengths and needsUtilizes effective interpersonal skills to engage youth, families, and informal and formal supports, including child welfare caseworkersAssuring care is delivered in a manner consistent with strength-based, family-centered, and culturally competent valuesGathers and inputs all data electronically for service progress tracking and quality assuranceComplete assessments, lead family team meetings, advocate for youth and family Qualifications: Bachelor's degree /or 3-4 years of experience dealing with high-risk youth in the juvenile justice systemYASI training (Youth Assessment Screening Instrument)At least two years of experience coordinating care, working with youth and families on a direct service levelAbility to work varying hours, including evenings and weekends to accommodate family's needsStrong interpersonal skills and dedication to helping youth and families reach their goalsStrong attention to detail in the coordination of services and care Benefits, Full-time: Health, Dental, VisionLife InsuranceShort/Long-Term Disability options403(b) RetirementPaid Time off (PTO)Mileage Reimbursement (exclusive to position)Eligibility for Public Service Student Loan Forgiveness Youth Outreach Services is a drug-free workplace and compliance with this policy is a condition of employment and continued employment YOS is an E-verify participating employer COVID-19 Policy: In compliance with Illinois Executive Order, Youth Outreach Services is requiring all employees to be fully vaccinated against or an approved exemption.
