Social Security Card
Drivers License
Auto Insurance (if applicable)
Health Certificate (physical exam and TB results within 6 months )




Training Certificates in:
HHA/ CNA License
BLS/CPR Card
Alzheimer's Disease (2-hour course) Approved by the Department Of Elder Affairs.
Assistance with Self-Administered Medications (2-hour course)
Domestic Violence (2-hour case)
HIPAA (2-hour course)
HIV/AIDS (4-hour course)
OSHA/Bloodborne Pathogens (2-hour course)
