Licensed Practical Nurse - LPN Essential Functions: Conduct Home Health Aide skills trainings, competency evaluations, HHA introduction and 90 day evaluationsComplete initial client assessment, plan of care, 90 day reassessments, and ongoing evaluations of the GAFC settingPartake in both care plan creations and reviews, along with discharge plansAssess and address motivational, psychosocial, and environmental concernsMaintain and update client charts, medical records and submit all relevant information to authorized organizationsCompleting notes corresponding with each on-site visit or encounter, or more often as the member's condition warrantsDocument health changes and communicate to the client's Case Manager and Program Manager in a timely mannerReview all paperwork and plans of care to ensure program compliance, updating information as neededPerforms any other additional duties that may be required by management at any time Licensed Practical Nurse - LPN Requirements: Travel is requiredValid drivers license and reliable transportation requiredLicensed Practical Nurse - LPN Massachusetts license requiredMinimum of 1 year experience in an acute or home health care setting or comparable work experience preferredFluency in both English & second language preferredCase management, social services, and/or community health worker experience preferredPast experience working with behavioral health population is a plusStrong communication, writing, organizational, and time management skillsSolution oriented with the ability to multitask
