Overview: NaviGuide Community Service Coordinator RiverWoods, in partnership with United Church Homes, is currently seeking a Community Service Coodinator.

The NaviGuide Community Service Coordinator is responsible for coordinating programs and services to assist client or residents in maintaining the highest practicable level of independence and physical, mental and psychosocial well-being in the New Hampshire area.
This is a remote position which would require the ability to travel around the service area as well as the ability to work onsite in Exeter, Manchester, or Durham as needed.
This is a full time position working for United Church Homes.
Responsibilities: General:Assists and educates clients or residents and families to acquire and utilize community services which may be necessary to maintain a self-reliant lifestyleActs as a liaison between community agencies, service providers and clients or residents; seeking out new services and identifying low-cost providers and/or negotiating discountsMonitors the delivery of services to clients or residents to ensure they are appropriate, timely and satisfactoryMaintains a resource directory of available community servicesMaintain professional relationship with all clients or residents in the housing community to identify areas of need and make referrals to community agencies as necessary.Maintains resident files, including but not limited to intake information, assessment, service referral and termination information, quarterly review, and follow-up, human or civil rights violations and resident, family and provider meeting or communication notesQuarterly Quality Service reviews and follow-upSubmits all required reporting data within specified timeframesNetworks with service providers to maintain necessary relationships and pursues avenues for additional services through private, local, state and federal sourcesThis position may require some travel for conference, trainings, and other company eventsResponsible for growing client portfolio through outreach, networking, and educating organizations about the Navi Guide program.Performs all other duties as assigned or directed NaviGuide Community Service Coordinator:Assess client needs to establish individual service planAct as family support system and provide any needed community resources for caregiversAbility to deliver home visits during inclement weatherMaintain client, family and provider meeting and communication notesAbility to commute daily for client home visitsThis position requires daily travel to client homes and community service providers, as well as any trainings, conferences, and other company eventsAll essential functions listed under the General essential functions Qualifications: Bachelor's Degree in social work preferredPrevious demonstrated history of sales experienceTwo (2) or more years' experience working with social service delivery to elderly population.
Working knowledge of resources available in geographic location serving the community.Must be able to proficiently operate a computer, the Internet, Microsoft Office, other relevant software, and basic office equipmentMembership and familiarization with American Association of Service Coordinators helpful Other Requirements: Proven experience in service management or facilitation, including organizing, problem-solving, and advocating; Must be able to read, write, understand and speak the English language; Must have strong interpersonal and communication skills; Must be able to work closely with residents, family members, legal representatives, and housing community staff; Must function independently, have a high level of flexibility and personal integrity; Must be able to adhere to all terms and conditions set forth in the United Church Homes Employee Handbook Equal opportunity employer.
