Nurse Care Manager

East Bay Community Action Program (EBCAP) is seeking a full-time Nurse Care Manager for our Health Center at 100 Bullocks Point in East Providence, RI. We are looking for a highly professional and motivated individual who enjoys working with a multi-disciplinary team in a community health setting.
The successful candidate will provide comprehensive screenings, assessments, care coordination services, disease education, and self-management support to specific patient populations. Nursing care is performed in both office-based and community-based settings, depending on the needs of the patient.
The successful candidate must have a minimum of an Associate's Degree in Nursing or equivalent, and be licensed as a Registered Nurse (RN) by the RI Department of Health. Two or more years of experience in a community health setting, acute care, or emergency/urgent care working with primary care providers to coordinate care and disease management is required. Previous experience in an electronic medical record (EMR) is preferred; however, training is provided in EBCAP's EMR which is NextGen.
