This unwavering focus on team culture and individual development is the cornerstone of our ongoing achievements.About the roleThe Nurse Practitioner, with a specialization in primary care, will collaborate closely with the Medical Director to deliver superior clinical services across IntraCare Health Center locations. Their responsibilities include directing, implementing, and providing patient care plans at the clinics. They will offer general preventive care, perform check-ups, treat acute and subacute conditions, order laboratory tests, and prescribe medications for patients.Clinical direction and oversight will be provided by a Physician.What you'll doConducts physical exams and observes patientsDocuments patients' medical histories and symptomsDevelops patient care plans and enhances existing onesOrders, administers, and interprets diagnostic testsMonitors and operates medical equipmentDiagnoses health issues and develops patient care plansDispenses medications and other treatmentsIdentifies changes in patients' conditions and adjusts treatment plans accordinglyCollaborates with healthcare professionals and familiesEducates patients and their families on illness and injury management and preventionPrescribes pharmacological and non-pharmacological treatment methodsAssists with patients' transitions across healthcare settings, such as admissions, transfers, and dischargesSupports staff, patient, and family decision-making through educational resourcesOversees and reports on the start-up status of clinicsProvides support to additional clinic locations as necessary and directedPerforms any other assigned dutiesQualificationsSkills and Abilities Required:- Exceptional people management skills.- In-office clinic experience required, family medicine preferred.- Superior written and verbal communication skills.- Ability to multitask and adhere to deadlines.- Proficient in computer usage.- Outstanding organizational and time management skills.- Keen attention to detail, robust character, and integrity.- Solution-oriented with excellent problem-solving abilities.- High level of professionalism and confidentiality.- Independent worker, capable of making sound decisions and knowing when to escalate issues.Education and Experience:- At least two years of experience in family medicine as a Nurse Practitioner (NP).- Graduated from an accredited nursing school.- Completed an accredited Nurse Practitioner program.- Current Registered Nurse license in the State of Arizona.- Current APRN/Nurse Practitioner license in the State of Arizona.- Board Certified in adult primary care or family practice.- Enrolled Medicare provider with an active PTAN number.- Enrolled AZ Medicaid provider with an active TPI number.- Holder of a current Basic Life Support (BLS) provider card.- Holder of a current Advanced Cardiac Life Support (ACLS) provider card, or willing to obtain it upon hire.- Current DEA License, or eligible to obtain one upon hire.- Geriatric care experience is an advantage.- Prior experience with Athena EMR is advantageous.- Strong computer proficiency, including Word, Excel, and PowerPoint.
