Other duties may be assigned.* Obtains patient's medical history, particularly as it relates to the patient's present condition, from the patient or family member.* Performs physical assessments to identify patient needs.* Evaluates the patient's family dynamics, home situation and environment.* Evaluates the patient's condition to determine what type and frequency of services are required and obtains physician's orders for same.* Establishes, maintains, and/or revises the patient's plan of care.* Evaluates the effectiveness of the plan and makes revisions based on patient's changing needs.* Informs physician of changes in the patient's condition, plan of care and/or frequency and type of services needed.* Consults and coordinates care with other active caregivers.*JOB REQUIREMENTS*Graduate of an approved school of nursing. Previous home care experience preferred. Must possess a current license to practice in the State as an RN.Must be able to read and interpret documents and manuals. Ability to write routine reports and correspondence. Ability to speak effectively before patients and their families.
