The Program Supervisor II is responsible for the supervision of staff and resources and for promoting a healthy environment. The position supervises the Rapid Re-Housing (RR) Case Managers , ensures compliance with safety standards and regulatory requirements, and is responsible for the safety of all residents, staff, and visitors. The supervisor, as a part of the leadership team, responds to crisis situations with RR Clients in coordination with another program leadership.
Upholds and models the CCSWW Mission, Insights, Key Action Steps and Principles of Leadership. Supervision: Supervises and reviews the performance of the RR Case Managers. Maintains appropriate levels of confidentiality. Sets Clear and Specific Expectations: Regularly review job description(s) & work assignments; provides ongoing & regular feedback to staff. Holds employee(s) supervised accountable for fulfilling job descriptions; ensures that responsibilities & work assigned are completed with consistent high quality & achieve desired results. Regularly evaluates employees in a constructive, timely & affirming manner, consistent with CCSWW philosophy of employee development. Takes appropriate action when employee performance does not meet expected standards. Review and address workplace safety issues. Provides job specific orientation & training. Commit to, model and assure zero tolerance for discriminatory behavior. Develop and ensure the safekeeping of a workplace that values and supports a culturally and ethnically diverse work environment. Ensures data entry into HMIS are completed within required timeframes. Notify Appropriate system of program vacancies. Receive and track referrals from Coordinated Entry. Keep FHN leadership informed about RR program performance compared to contracted goals.   Maintains Case Management case load: Provides direct services to clients including creating client-driven Housing Stability Plans. Support residents with barriers to their housing stability. Completes intakes with incoming clients to verify program eligibility. Completes all required documentation for client records and billing systems. Maintains compliance with all appropriate contracts and agency policies. RR Responsibilities: Respond to crises and provide support to RR Case Managers as needed. Act as point person to establish positive and proactive relationships with Landlords, Property Management and key community partners. Inform Program Manager of developments and engage as appropriate. Oversee and provide behavioral management by ensuring Case Managers assist residents to resolve conflicts and make pro-social choices. Review Case Notes, Housing Stability Plans, and HMIS data and respond to issues ensuring Case Managers work with clients according to program design and values including progressive engagement, trauma informed care, and housing first. Ensure that any required documentation is completed and submitted on a timely basis. Review and approve all requests for Client Assistance payments and monitor spending in relationship to budget. Communicate any necessary recommendations for revision to security practices to peers and supervisor as needed to promote a safe environment and to support program and agency goals. Work collaboratively with other Family Housing Network and CCS/CHS staff to provide coordinated services to residents. Facilitate regularly scheduled meetings with all Case Management Staff including those from outside agencies. Ensure that appropriate safety and sanitation procedures are being followed by regularly inspecting the facility for cleanliness and safety and referring issues to Building Management. Create activities and facilitate a service-rich environment to promote residents' safety, stability, and personal growth. Ensure Program Director and appropriate CCS leadership are kept apprised and up-to-date on all relevant program/staff related information. Other duties as assigned. MINIMUM: MINIMUM: BA in social services or related field, OR AA with 2 years with relevant experience, OR 5 year's relevant experience.
1-year of case management experience. Experience in supervising, leading and/or coordinating direct services staff on a daily basis. Experience working in Permanent Supportive Housing Programs. Work with diverse population including people with active mental health and substance use disorder issues. Must have reliable transportation, valid driver's license and automobile insurance. Must have an acceptable driving record per CCS' driving policy (required to provide current full record-driving abstract). Support and uphold the mission, beliefs and values of Catholic Community Services and Catholic Housing Services and a knowledge of and commitment to working within the Social Teaching of the Catholic Church. Support and contribute to a creative, collaborative and respectful environment that promotes teamwork. Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations. Criminal History Background Checks are required prior to employment. PREFERRED: Master's Degree in social services or related field. Experience with program development and management.
