Job Summary:Under the direction of the Director of Public Health, the Registered Dietitian (RD) will be an integral member of the Special Diabetes Program for Indians (SDPI) team, providing comprehensive, culturally sensitive nutrition assessment, education, and support to urban Native American communities. The RD will develop and implement individualized and group nutrition plans, contribute to program planning and evaluation, and work collaboratively with healthcare providers to improve diabetes management and overall health outcomes for the population served. The RD will collaboratively plan and conduct community education and prevention activities, including the National Diabetes Prevention Program and other events. The RD will serve as a resource for other agency staff in nutrition-related matters.Responsibilities: Nutrition Assessment and Counseling: Conduct thorough nutrition assessments for patients with diabetes and other related conditions. Develop, implement, and evaluate individualized nutrition care plans based on assessment findings. Provide one-on-one and group nutrition counseling to patients, focusing on diabetes management, cardiometabolic conditions, weight management, and overall wellness. Provide diabetes self-management education within the scope of practice and experience of the RD. Program Development and Implementation: Develop and deliver culturally relevant food, nutrition, and diabetes education programs and workshops for individuals and groups. Collaborate with SDPI team members to design and implement strategies to enhance program effectiveness and reach. Assist in the creation of educational materials, including handouts, presentations, and online resources. Clinical Collaboration: Work closely with medical providers both in-agency and outside to coordinate patient care and ensure holistic treatment approaches. Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as necessary. Data Management and Reporting: Maintain accurate and up-to-date patient records and documentation. Assist with the collection, analysis, and reporting of program data to monitor progress and outcomes. Prepare reports and contribute to grant writing and program evaluation efforts as needed. Community Outreach and Education: Engage with the urban AI/AN community through outreach activities and partnerships to promote nutrition and diabetes prevention. Foster relationships with community organizations to enhance program visibility and accessibility. Professional Development: Stay current with emerging nutrition science and diabetes management practices. Participate in continuing education and professional development opportunities relevant to the role.Minimum QualificationsBachelor's degree in Dietetics, Nutrition, or a related field from an accredited institutionRegistered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential from the Commission on Dietetic Registration (CDR) required. Utah state licensure required upon acceptance of position.Certifications/Licensure:Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential from the Commission on Dietetic Registration (CDR) required.
