The Registered Nurse Case Manager plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

RNCM Home Health requirements: 1. Current and valid California RN licensure.
2. One to two years of recent acute care experience in an institutional setting.
3. Basic Life Support (BLS) through the American Heart Association
4. Valid driver's license and auto insurance.
Preferred Qualifications: 1. Bachelor's degree with one to three years of home health care experience.
Pay Transparency: Range: $55.00 to $60.00 an hour.
