We value your ambitions and offer pathways for professional development that align with your goals.RN Case Managers oversee the entire care of the patient beginning to end while under the care of Residence Home Care.· Provide care wherever the patient calls home.· Manage other disciplines such as LPNs, aides and therapy staff to give the best care needed for each patient.· Ability to complete OASIS, recert, d/c, ROCs documentations within an EMR System. (WellSKY)· Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.Our RN Case Managers participate in a flexible on-call rotation with additional compensative.
