**Job Description:**
- Utilize evidence-based practice to support clinical interventions.
- Provide rehabilitative, skilled, and necessary treatment to patients with cognitive and/or physical functioning disorders, under the supervision of an Occupational Therapist.
- Administer procedures as part of the rehabilitation plan including ADL retraining, therapeutic activities, exercises, and modalities.
- Assess patient progress and communicate with the supervising Occupational Therapist to adjust treatments as necessary.
- Report effectiveness of treatment outcomes to the supervising Occupational Therapist.
- Provide consultation to patients, families, caregivers, and other service providers.
- Communicate effectively with the supervisor and other health team members regarding patient progress and treatment plans.
- Contribute to discharge summaries of services provided as per regulatory and clinical requirements.
- Document all required information in the electronic documentation system according to company policy.
- Accurately record treatment minutes per all regulatory and clinical requirements.
- Participate in required meetings such as patient care conferences and rehabilitation meetings.
- Engage in facility and company-required training sessions and conferences.
- Work collaboratively with the supervising Occupational Therapist to obtain necessary DME or supplies.
- Support and supervise students under the guidance of the Occupational Therapist.
- Adhere to NBCOT/AOTA and State Required practice acts, codes of conduct, and ethics during professional practice.
