Job Description:

- Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
- Utilize standardized assessments and evidence-based practice to support clinical interventions.
- Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders in accordance with regulatory and clinical practice requirements.
- Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and the use of supportive and assistive devices.
- Assess patient progress and adjust treatments accordingly in the clinical record as per regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness, modify services in relation to patient evaluation, and make referrals in relation to outcomes.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
- Communicate effectively with supervisors and other healthcare team members regarding patient progress, barriers, and treatment plans.
- Provide comprehensive discharge summaries of services provided per regulatory and clinical requirements.
- Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy.
- Accurately record treatment minutes for services provided per all regulatory and clinical requirements.
- Participate in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings.
- Engage in facility and company-required training, in-services, and conferences.
- Obtain any necessary DME or supplies per regulatory and clinical practice requirements.
- Supervise and support occupational therapist assistants, rehabilitation aides, and/or students.
- Adhere to state-required practice acts, code of conduct, and ethics during professional practice as an Occupational Therapist.
