- Resume and license verification attached
- Covid test 72 hours prior to start date with results issued 48 hours prior to start date

**Job Description:**
- Ensure the prescreen form is completed and attached to the candidate's application.
- Attach resume and license verification to the application; failure to do so will result in candidate being declined.
- Experience in Skilled Nursing Facilities (SNF) is preferred for this position.
- A Covid test must be taken 72 hours before the start date, with results available 48 hours before the start date.
- Complete the credentialing process, including:
- State License Verification
- Submission of references (3 required)
- County Criminal background check (7 years for all counties lived and worked)
- Federal Criminal background check (7 years or more)
- Multi-State Criminal Search (7 years or more)
- Nationwide Sex Offender Search (7 years or more)
- HHS/OIG Search
- GSA Search
- Social Security Number Trace
- 2 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray
- SAM Search
- Washington State Background with Fingerprints (facility run)
