Care may involve assessments, administering prescribed treatments and therapies, care planning, communication with other members of the patients' multi-disciplinary team, supply management, emergency management, coordination of care, health promotion and teaching and training family members.Essential Job FunctionsAssessing, diagnosing, and preventing language, speech, and swallowing disorders for children, adolescents and/or adults.Design plan of care utilizing functional, measurable and attainable goals, based upon interpretation of evaluation results and patient and/or family goalsProvide a written evaluation defining Speech Therapy needs, goals and expected outcomes.Maintains plan of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determine discharge care plans as appropriate.Participate in coordination of services (including discharge planning), with health care team, physicians, parent/guardians, nurses, and others.Documents in accordance with Speech Therapy Standards of Practice, Aveanna policies and procedures and Regulatory Agency guidelines.Documents effectively resulting in no technical or clinical denials subsequent to review of documentation by payersMaintain effective communication between staff, health care team members, and family.Mentor peers, new graduates and serve as Clinical Instructor for speech therapy students as neededSupervise CFY's, Speech-Language Pathology Assistants and/or volunteers as assignedMaintain patient and company confidentiality.Participate in program development, marketing, and continuing education for skill development and competency.  Attend and participate with in-services as indicated.Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.RequirementsMaster's degree in Speech-Language Pathology from an ASHA accredited college/universityCurrent and unrestricted SLP license to practice in the state of practiceCurrent CPR certificationNPI numberDemonstrated proficiency in clinical assessments, documentation and compliance with policies and proceduresAdditional state specific requirements:Continuing Education as required by statePreferencesOne (1) year of recent experience as a Speech Language Pathologist in an acute care settingPediatric experience preferredOther Skills/AbilitiesAbility to adhere to confidentiality standards and professional boundaries at all timesAttention to detailTime ManagementEffective problem-solving and conflict resolutionGood organization and communication skillsPhysical RequirementsMust be able to speak, write, read and understand EnglishMust be able to travelMust be able to lift 50 poundsProlonged walking, standing, bending, kneeling, reaching, twistingMust be able to sit and climb stairsMust have visual and hearing acuityMust have strong sense of smell and touchMust be able to sufficiently reposition patients and move equipment without assistanceMust be able to appropriately respond physically and mentally to emergency situations in the home or outpatient facilityEnvironmentMust be able to function in a wide variety of environments which may involve exposure to allergens and other various conditionsPossible exposure to blood, bodily fluids and infectious diseasesOther DutiesPlease note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
