Position Summary

The Admission/Discharge Registered Nurse coordinates care between members of the healthcare team.
The Admission/Discharge nurse facilitates the admission process.
The Admission/Discharge nurse facilitates the discharge by coordinating discharge instructions between the physicians, providing for patient education, and assuring medication reconciliation.
The Admission/Discharge nurse plans, implements and evaluates care based on physician orders, current established Standards of Care, the Nurse Practice Act and the St. Catherine Hospital Policies and Procedures.
The Admission/Discharge nurse collects patient/health data to determine a nursing diagnosis and identified expected patient outcomes for nursing interventions.
The Admission/Discharge nurse prepares discharge instructions to assist team leaders to provide patient education, assure medication reconciliation, and coordinate instructions among the physicians.
The Admission/Discharge nurse also prepares required documentation to facilitate transfers to another unit, or another facility.


Job Responsibilities Assesses the biophysical, psychosocial, environment, self-care, educational and discharge planning needs of the patient on admission.
Identifies risk factors/patient safety issues on admission and takes appropriate action Assures accurate medication reconciliation.
Facilitates timely discharge by assisting with coordination among physicians Communicates with patients, families and/or significant others in an effort to individualize patient's plan of care to them.
Education/Experience Graduate of an accredited school of nursing.
Currently licensed as a professional nurse by the State of Indiana Health Professions Bureau.
A BSN degree is desirable.
Successful completion of the Community Healthcare System's Personnel and Nursing Orientation Programs.
Obtains and maintains current certification in BLS, and other certifications as required in specialty areas.
