POSITION SUMMARY
The purpose of your job position is to assist the facility with a timely patient admission per the facility's established policies and procedures and in accordance with current federal, state and local guidelines, and as directed by management and/or the Administrator.
The Admissions Nurse will treat each patient fairly, kindly, and show them dignity and respect.
They will communicate clearly, empathetically, and effectively when speaking to patients, family members, visitors, all staff and governmental agencies.
They will be courteous to families, visitors and all staff.
They will maintain a positive and calm disposition.
They will work cooperatively with all departments and multidisciplinary teams.
They will be committed to always doing the right thing.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Overseeing inquiries from physicians, hospital discharge planners, etc.
and responding to all referrals within 15 minutes of receipt
Maintaining an accurate record of available beds
Monitoring the patient's care plan
Interviewing patients/guardians/sponsors and obtaining required admissions information and signatures
Notifying nursing department when the patient arrives so they can be escorted to their room
Helping with patient admission orientation program
Communicating any admissions problems immediately to a nursing supervisor
Updating a list of all patients/residents and assigned room numbers
Providing excellent customer service to potential residents and family
Making calls to physicians and hospitals as required
For PDPM compliance, ensuring all new admissions to the facility have proper documentation discharge summary, therapy notes, nursing notes, and medication list
Assisting the MDS Coordinator or Case Manager with scheduling patient assessments
Keeping updated with Medicare/Medicaid regulations governing admission and discharge requirements

REQUIREMENTS
Education / Licensure
Nursing Degree from accredited school or college
Valid LVN or RN license in good standing
Valid CPR and BLS card
Qualifications / Experience
Minimum of 6 months experience with admissions preferably in a long-term care setting
Must be able to speak, read, write and comprehend the English language
Proficiency with PCC a plus
Working Conditions
May encounter frequent interruptions
May be involved with residents, family and government agencies
May be requested to work beyond scheduled working hours at times
May be subjected to odors, dust, disinfectants, tobacco smoking or air contaminants
May be exposed to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses
May be exposed to or handle hazardous chemicals
Physical Requirements
Must be able to move intermittently throughout the day
Working throughout the nursing areas
Ability to read fine print on tablet, progress notes and/or medical labels
