SUMMARY:

Both the Advanced Registered Nurse Practitioner and the Physician Assistant fall under this job title. The Advanced Practice Provider, a registered professional with advanced training, functions as a collaborative member of the health care team in the delivery of comprehensive health and health maintenance to patients in primarily, but not exclusively, ambulatory settings. Primary health care includes care of patients with acute episodic or chronic illnesses, emergency care, rehabilitation, preventive health measures, health maintenance and health education services. They will be required to participate as part of the on-call team which will be arranged so that appropriate coverage is available 24- hours a day for all specialties. The Advanced Practice Provider may perform medical services when such services are rendered under scope of their license as allowed by the organization in their privileging and credentialing file.

ESSENTIAL FUNCTIONS:

Patient Management:

Elicits histories and conducts physicals.

Orders appropriate labs, EKGs, x-rays and be able to recognize abnormal findings.

Prescribes and dispenses medications and treatments following the appropriate standard of care guidelines.

Initiates requests for consultation or referral with appropriate professionals, clinics, or other health care resources as indicated.

Assesses and triages patients.

Refers or seeks consultation with physician when a patient has a condition for which the Advanced Practice Provider is not credentialed.

Patient Education:

Educates patient on all aspects of health self-maintenance.

Provides short-term, individual, or family teaching on diet, weight control, adolescent birth control, and sex education on a consult basis.

Assists with development and implementation of patient educational materials.

Electronic Health Record (EHR):

Accurately and efficiently enters documentation into the EHR.

Ensures all EHR documentation is completed accurately and timely.

Cosigns charts as required in a timely fashion.

Cosigns charts as required timely.

Ensures all charts are closed out in Athena timely.

Demonstrates basic understanding of the EHR

QI/QA

Actively participates in quality improvement projects.

Participates in ongoing peer review of medical records, active participation in committee assignments, and QA/QI activities.

Participates in and presents Continuing Medical Education activities.

Complies with Center policy and procedures; promotes and complies with NCQA PCMH standards of care.

Participates in program required committees.

Assists in identification, evaluation, and intervention of quality issues in the clinical setting.

Leadership

Maintains ethical and professional standards deemed appropriate by peers.

Supervises nursing and clinical staff activities.

Provides supervision, education, and direct assistance when appropriate to Novice Physicians, Advanced Practice Providers and student providers, as reasonable, considering the provider's training and capabilities.

Performs other related duties as indicated or when requested by the Chief Medical Officer or their designee.

Assists with management of patient flow to assure effective and efficient care is provided to all patients

Covers for other providers within the scope of licensure as needed.

Other:

Appropriately applies patient evaluation and management processes to clinical situations.

Always maintains use of universal precautions.

Completes Daily/Day end activities as assigned.

Completes all required trainings and attends meetings as assigned.

Protects/observes patient confidentiality per policies and procedures.

Recognizes and responds appropriately to violent/abusive situations, bomb threats, fire and emergency situations.

MINIMUM QUALIFICATIONS: (EXPERIENCE/SKILLS/ABILITIES/EDUCATION/CERTIFICATIONS):

Graduation from an approved school of medicine or osteopathy

Three (3) years of approved residency in their specialty field.

Must maintain required CME credits.

Possession and maintenance of a current Florida license to practice medicine

Possession and maintenance of Board Certification in area of specialty when appropriate

Good written and verbal communication skills.

Ability to effectively interact with physicians, patients, and other staff members.

Effective use of Electronic Health record. Possesses intermediate typing skills.

Demonstrated knowledge of proper, safe, and efficient use of current office equipment/software.

Demonstrated knowledge/ability to use Office 365 programs, including Outlook and Word.

PREFERRED QUALIFICATIONS: (EXPERIENCE/SKILLS/ABILITIES/EDUCATION/CERTIFICATIONS):

Preferred knowledge primary care, cultural diversity, performance improvement and coding.

CPR certification recommended

Experience or basic understanding of immunizations, laboratory services, dental programs and 340B drug program is recommended.

PHYSICAL DEMANDS:

While performing the duties of this job, the employee is regularly required to stand; walk, often for prolonged periods; use hands to finger, handle, or feel; reach with hands and arms; stoop, kneel, crouch, or crawl; and see, speak, and hear. The employee must regularly lift and/or move up to 10 pounds and frequently lift and/or move up to 25 pounds.
