The On-Call Weekend Registered Nurse is responsible for developing and initiating the plan of care, providing preventative and rehabilitative nursing procedures, and offering patient and caregiver counseling and education.
You will collaborate with the interdisciplinary team to assess and develop individualized care plans and communicate with physicians and other healthcare practitioners as needed.
Additionally, you will identify discharge planning needs and participate in quality assessment and performance improvement programs.
Duties & Responsibilities: Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s) Provide preventative and rehabilitative nursing procedures as ordered by the physician Offer patient, caregiver, and family counseling and education Prepare clinical notes and communicate with the physician and other healthcare practitioners Identify discharge planning needs and implement prior to patient discharge Communicate with community health related persons to coordinate the care plan Participate in on-call duties as defined by the on-call policy Supervise and evaluate home health aide care provided every two weeks Job Requirements: Graduated from an approved school of nursing and licensed in the state where practicing Minimum of two years of experience, with preference for experience in public health or home care nursing Maintains a current CPR certification Valid driver's license and insured automobile in good working order Self-directed and able to work with minimal supervision Excellent observation, problem-solving, verbal and written communication skills Ability to organize and prioritize workload independently Prolonged or considerable walking or standing, able to lift and transfer patients, and perform required nursing skills Schedule is Friday 5pm to Monday 8am; then Monday 5pm to Tuesday 8am Location: Parker and Big River Area
