Major/Essential Functions
Assist in examination, treatment, and discharge of patients under the direction of a physician.
Establish rapport with individuals in targeted communities that are being asked to participate in the project. Work with the TTUHSC El Paso department of Psychiatry, community organizations such as the opportunity center. Inform individuals about the ability and resources for mental health. Must meet established goals and deadlines. Is expected to meet with about 20-30 participants per week.
Measure, collect, and record patient's vital signs (i.e., pulse rate, oxygen, temperature, blood pressure, weight, and height), and prepare patients for examinations and tests/procedures.
Attend weekly meetings to report back to the team on the outreach initiative and update material about patients. Must be available to attend meetings and be prepared to discuss as instructed by Project Director.
Prepare and administer routine medications (oral, rectal, topical, nebulized, intramuscular, or subcutaneous routes); and injections (excluding controlled substances) as directed by the physician as applicable to assigned clinical department. Identify, monitor, and report adverse reactions.
Maintain confidentiality and patient privacy at all times. Verify and ensure patient data is accurately documented within electronic systems, and distributed via manual processes.
Schedule follow-up care (i.e., appointments, referrals, testing) as requested.
May perform clinical support duties including but not limited to: phone triage, filing, front-desk reception, data entry, and/or patient registration as requested.
May be required to travel as a member of the multi-disciplinary healthcare team to provide patient care at TTUHSC EP outreach clinics as applicable to assigned clinical department.
Collect data using Microsoft surfaces and report back to the community engagement team. Work with the team to document recruitment and education activities. Data collection must be organized, consistent, accurate and readily available for review.
Collaborate with community agencies and providers to ensure access for service in line with Individual Treatment Plans. Maintain positive community relationships to ensure accurate referral access. 100% of all services will have proper authorization on file.
Adhere to all TTUHSCEP policies, procedures, and processes.
