As an Assessor Reviewer II, you will be responsible for face-to-face, primarily in-person assessments of vulnerable populations and individuals entering or living in nursing facilities in Colorado. Persons being assessed may reside in the community or facilities. You may also complete review decisions regarding services needed for the person(s) to be successful in the community and appropriate placement needs. You may also serve as a mentor or preceptor for new staff, assisting in the formal training of assessors, completing quality assurance competency reviews, and may be involved in process improvement initiatives.
We are looking specifically for individuals in Northern Colorado, Denver Metro, and Southern Colorado North (Ft. Collins, Loveland, Greely, and surrounding areas)
Denver Metro (including Boulder/Broomfield, Aurora, and south metro Denver – Littleton, Douglas County)
Southern Colorado (including Colorado Springs, Pueblo, and surrounding areas)
This is a home-based position that will primarily travel to cover a geographic area to complete assessments. As business needs and assessment volumes vary, additional travel outside of the primary geographic area may be required. All assessors will be completing assessments primarily in the front range of Colorado with a possibility of some travel to rural areas.
\n Essential FunctionsPerform Pre-admission and Resident Review (PASRR) Level 2 face-to-face and/or as needed, virtual assessments of persons entering or living in nursing facilities.Collaborate with health care providers and ancillary service providers, family, or others to assess the individual appropriately.Document Level 2 evaluations in Telligen's web-based portal.Attend quality assurance competency training and quality improvement efforts as assigned.May support other assessors in being trained as needed. RequirementsMaster's degree in Behavioral Health OR Licensed RN with experience/education with behavioral health/mental health populations.Current license to practice in the State of Colorado, must include one of these licenses: LCSW, LPC, LMFT, or LPRN. Some contracts may require certified QMHP or LMSW.A valid driver's license is required for all roles.License endorsement in other states may be requested.QIDP or sufficient experience and education working with individuals with IDD and/or SIS assessment experience is a plus.
\n$80,000 - $95,000 a year
The salary for each individual will be determined by factors such as location, education, experience, and internal equity.
