Job Description
Under the direction of a licensed nurse, a certified nursing assistant performs daily personal care including dressing, grooming, and maintaining proper clothing and hygiene, food preparation, cleaning, recreational activities, and housekeeping at residents' request.
The CNA's primary responsibility is to ensure the residents' quality of life by meeting their psychosocial and personal needs in a safe and clean environment.
Duties and Responsibilities

Provides personal hygiene (bath, shower, shampoo, shaving, dressing, elimination needs, facial hair clipping, nail filing oral hygiene other personal care).Assists with hearing aides and dentures when needed.
When needed, performs safe and proper transfers, assists with ambulation by using assistive devices such as gait belts, walker, and wheelchair.
Accurately weighs residents as assigned in a timely fashion.
Reports observations and changes in resident condition.Provides scheduled toileting and incontinent care to residents who need assistance.
Provides physical comfort and a safe environment.
Follows all safety and security policies and procedures; performs all tasks to assure resident and personal safety and the protection of co-workers and facility property.Promptly and courteously answers residents' requests for assistance.Follows infection control, blood borne pathogens and hazardous materials policies and procedures.Follows residents' plan of care and respects residents' choices.Assists with dining room duties, including serving the residents their meals, delivering trays and assisting with filling out menus.
Completes documentation in Matrix care in an accurate and timely fashion.Communicates with nurse any changes in resident care needs not reflected on the plan of care.
Assists residents with self-administration of medications.Assists residents with keeping living areas and property clean and operational.Supports, leads, and participates in a resident-centered recreational activity program.Cleans common areas and assists with light housekeeping including laundry and ironing.Provide companionship including walks outdoors with residents as weather permits.Accompany residents to appointments as needed.Provides administrative support including filing, logging in-services, tracking missed punches and other duties as assigned.
Cover assignments as needed for callouts both in the main building and in the Dogwood neighborhood.
Coverage of Caleb 1:1 assignments when needed.
As necessary fulfill overnight requests to provide support.
Performs other duties as assigned. Job Qualifications Current certification as a Certified Nursing Assistant in the State of Connecticut.Must be eligible for Certified Nursing Assistant registration in the State of Connecticut.Must have effective communication skills.Must have the ability to read, write and speak English.Must have a high school education or equivalent preferable.Must be able to safely perform the essential job functions with or without reasonable accommodations.Must have computer skills with the ability to document in the electronic medical record, in-services and communication via email or text. 3rd: 11:00pm - 7:30am
Pay Rate
$19.00 p/h + shift differential ($1.50 p/h weekday evenings, weekday nights & weekend days, $2.00 p/h weekend evenings & weekend nights)
