The Behavioral Health (BH) Specialist focuses on managing a population of individuals by working collaboratively with the BHH team to assess needs, plan for care, and support progress of individuals served as part of the Behavioral Health Home (BHH) model.


Compensation Range:

The starting pay for this position is $44,000 annual salary.

Screen individuals for mental health, substance use disorders, and environmental concerns as appropriate.
Develop and modify a person-centered behavioral health home care plan for enrolled individuals by working to identify chronic behavior, discuss impact, and develop improvement strategies and specific goal-directed interventions.
Refer and coordinate with primary and specialty care physicians, internal and external mental health professionals, and other healthcare providers as appropriate.
Conduct brief interventions for individuals with behavioral health problems.
Meet regularly with the care team to plan care and discuss cases, exchange appropriate information with team members in an informal manner as part of the daily routine of the clinic.
Support primary care providers in identifying and behaviorally intervening with individuals served.
Assist individuals in identifying and gaining access to formal and informal supports available in their community and match specific supports and interventions to the unique needs of individual while recognizing the importance of friends, family, and community relationship.
Develop and maintain relationships with community based mental health and substance use providers.
Provide education on health conditions and strategies to implement care plan goals including both clinical and non-clinical needs.
Utilize motivational interviewing strategies and a trauma-informed approach to engage with and provide service to individuals.
Complete all required documentation completely, accurately, and in accordance with CNS Meet direct service productivity and documentation standards per agency and program expectations.
May provide transportation to individuals served as needed.
Fulfills mandatory education requirements of CNS including evidence-based programs, internal training, and community education.
Maintain compliance with quality care initiatives including Meaningful Use (MU), Medicare Merit Based Incentive Payment System (MIPS), Commission on Accreditation of Rehabilitation Facilities (CARF), and Certified Community Behavioral Health Clinic (CCBHC) requirements.
Ensure that the CNS Healthcare standards of service are applied to interactions with individuals served, guests, and staff.
Assume responsibility for professional growth by current reading and attendance at educational programs and conferences.
Maintain knowledge and compliance with established policies and procedures, corporate compliance program, code of ethics, applicable federal, state, and local laws and regulations, HIPAA standards, and other regulatory programs.
Perform other related duties as assigned.
Qualifications: Required Qualified Mental Health Professional: At least a Bachelor's Degree in Social Work, Sociology, Counseling, Psychology, Criminal Justice, or other Human Services field Specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating and working with a person who has mental illness.
Working knowledge and understanding of mental illness, medications used in treating psychiatric disorders, and knowledge and experience with other interventions used to treat psychiatric disorders.
Ability to write clear, concise reports and the ability to formulate and implement an effective treatment plan.
Proficient with computers, Microsoft office products (including Outlook, Word, and Excel), and electronic health records (EHRs).

Ability to work closely with persons with mental or physical limitations.

Valid and unrestricted Driver's License Transportation or Automobile accessibility with current automobile/vehicle insurance meeting agency policy requirements.
Preferred A Master's Degree in social word, education, psychology, counseling, nursing, or closely related field State licensure: Limited-Licensed Bachelor's Social Worker (LLBSW), Licensed Bachelor's Social Worker (LBSW), Limited-Licensed Master's Social Worker (LLMSW), Licensed Master's Social Worker (LMSW), Limited-Licensed Professional Counselor (LLPC), Licensed Professional Counselor (LPC), Limited-Licensed Marriage and Family Therapist (LLMFT), Licensed Marriage and Family Therapist (LMFT), Temporary Limited-License Psychologist (TLLP), Limited-Licensed Psychologist (LLP), OR Licensed Psychologist (LP) Greater than one year of experience in treating and working with adults with severe mental illness (SMI) Ability to share and teach about the symptoms of substance use disorders that are similar to those of other medical and/ or psychological disorders and how these disorders interact.
Basic knowledge of chronic health conditions and understanding of integrated healthcare models and practices.
Prior experience in a community mental health setting or in-patient setting.
