**MAJOR RESPONSIBILITIES:**
The employee will be directly responsible for managing a caseload of individuals with chronic physical health, mental/behavioral health, and social needs. The primary goal of the employee will be to assist individuals in reducing avoidable emergency room visits and hospital admissions through the provision of care management services.
**DETAILED RESPONSIBILITIES:**
* Provide services to the individual through linkage and referral to appropriate community services.
* Coordinate services with community providers, hospitals, and family members.
* Complete comprehensive assessments, develop with the client care plan goals
* Monitor and document services provided and progression of goals.
* Capture engagement of clients in detailed progress notes.
**QUALIFICATIONS:**
* Computer knowledge, including but not limited to Windows, Microsoft Office.
* Can work as part of a team with co-workers and with other colleagues as necessary.
* Cultural sensitivity and the ability to relate/work with diverse groups, community agencies, schools, and the general public.
* Excellent verbal and written communication skills, including strong interpersonal and organizational skills, focusing on detail.
* Ability to follow directions and communicate well (orally and in writing)
* Bi-lingual (Spanish) Preferred.
**EDUCATION/TRAINING/EXPERIENCE:**
Bachelor's degree in one of the fields listed below:
Social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech, and hearing or other human services field.
OR
Credentialed Alcoholism and Substance Abuse Counselor (CASAC) with two years of experience in providing direct services to people with Serious Mental Illness, developmental disabilities, or alcoholism or substance abuse; or linking individuals with Serious Mental Illness, developmental disabilities, or alcoholism, or substance abuse to a broad range of services essential to successful living in a community setting (e.g., medical, psychiatric, social, educational, legal, housing, and financial services).
A master's degree in one of the listed education fields may be substituted for one year of experience.
**REQUIRED:**
