Over the next 3-5 years, Tepeyac Community Health Center seeks to double patient visit capacity, in addition to the inclusion of pharmacy services.*JOB SUMMARY:* The Physician Assistant or Nurse Practitioner is an integral part of the care team and will work collaboratively with all patients and staff. They will make the patient the primary focus of their actions alongside developing and sustaining productive and compassionate patient relationships. They will contribute to the health care team by influence, innovation, and by modeling effective integrated care and exemplary service. MINIMUM QUALIFICATIONS: ** Education for Physician Assistant: Bachelor's Degree required.* Graduation from a program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA).* Education for Nurse Practitioner: Master's Degree from an accredited School of Nursing.* Experience: One year of experience in family medicine required. Three years of experience preferred.* Language: Oral and written fluency in English and oral proficiency in medical Spanish required (fluency preferred).* Current or eligible for unrestricted licensure in the State of Colorado.* Active DEA licensure or eligible for DEA licensure (new graduates should obtain within a year of hire).* Valid national and state of Colorado certification as Nurse Practitioner or Physician Assistant and in compliance with requirements.* Must provide proof of 100 hours of CME every two years.* Ideal Candidate: Strong initiative and passion to advocate and provide healthcare to the underserved.*B. ESSENTIAL DUTIES & RESPONSIBILITIES: ** Obtain a pertinent medical history, perform an appropriate physical examination, and review relevant laboratory or radiologic (or other) studies to assess the patient's medical, physical, psychological, and relevant socio-economic status.* Formulate an accurate and appropriate diagnosis based on elicited data and include differential diagnoses as appropriate.* Order and administer appropriate medications within the limits of the appropriate statutory restrictions.* Provide care with attention to safety, efficiency, and cost effectiveness.* Plans are appropriately prioritized and adapted to the patient and circumstance.* Perform procedures within the limits of their expertise and in accordance with the State Regulations and Organizational Policies.* Educate and counsel patients and their families regarding their treatment plan, including disease prevention and health promotion.* Promote the availability of culturally sensitive health and resource information that is evidence-based, patient centered, and enables informed choice.* Document all examination data, diagnostic impressions, appropriate codes, and other patient care activities in the medical record, including the name(s) of any consulted physicians or other providers.* Completes required documentation thoroughly, and in a timely manner. Prepares and maintains complete and accurate charts, case files and records.* Observe the principles and techniques of Universal Precautions.* Conduct telephonic and virtual encounters as appropriate.* Attend and participate in staff meetings.* Participate in clinical coordination, quality, and process improvement efforts which may encompass protocol development, strategic planning, and other administrative tasks as necessary to support practice environment.* Participate in the training, mentoring, and/or clinical education of new employees, colleagues, and students.* Manage assigned in-basket tasks in a timely manner.* Take call as directed by supervisor.* Take necessary and appropriate actions in emergency situations.* Perform other clinical/administrative duties as directed.*C. KNOWLEDGE, SKILLS & ABILITIES:** Ability to execute work plans independently and with flexibility, with minimal supervision.* Strong organizational skills. Benefits include medical, dental and vision insurance, a flexible spending account, a 401k, disability insurance paid for by Tepeyac, eight paid holidays, four weeks of vacation and 6 days of sick leave each year. Licensed providers receive time off and reimbursement for CME and reimbursement for the Colorado licensing renewal fee.
