Job Position: Bilingual LPN Care Manager
Location: Chicago, IL 60639
Description: Bilingual (English and Spanish) Role Requires 20% Remote and 80% in office
Pay Rate: $33.00/hr - $35.50 based on experience
Shift: Monday - Friday 8am - 5pm (one late shift a week).
Offering $33.00/hr - $35.50 based on experience Health, Dental, Vision, PTO offered JOB RESPONSIBILITIES Identifies, tracks and manages care for the highest risk patients Conducts initial care coordination screening eligibility and performs comprehensive risk assessment Works directly with patients/family members and providers to develop and implement an individualized and integrated patient care plan for each of the highest risk patients.
Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans Adapts practice to meet individual patient circumstances with regard to health literacy, deficits as well as cultural and linguistic differences, ensuring that effective communication is achieved, particularly where there may be barriers to understanding Utilizes motivational interviewing to gain cooperation from patients and families around the care management plan Work alongside medical home staff at clinic site Required Experience/Licensure: 3-5 years of experience with 1-2 years in disease management or care management as RN, LPN or Social Worker Medical Licensed required (LSW, LCP, LPN, RN) A completed Master's degree in a human services field for social work Must be open to schedule including day/evening and weekend shifts Required Skills:
Strong analytical skills; detail and solution oriented; ability to work independently; excellent written and oral communication and presentation skills; and ability to use spreadsheet or database software; Proficient computer skills including, but not limited to, the ability use the following on a daily basis: Web-based applications, Email, Microsoft Word.
