Health benefits—opt in to medical, vision and dental coverage, and telemedicine Paid time off—to take care of yourself and your family Responsibilities: Work closely with the Care Management team to deliver an excellent experience at every interaction Receive and process calls from potential clients, care providers, family members, hospital staff and other medical professional staff Present a professional appearance and promote a positive work environment Coordinate with the Training Specialist and Scheduling staff to help build new hire schedules Comply with all agency policies and procedures, ensuring data accuracy and confidentiality Develop home safety plans. Identify client physical, nutritional and hygiene supports to include transfer and mobility. Perform initial assessments for Clients with external Case Managers (Medicaid, VA, OPI, ODDS, etc.). Qualifications: Minimum of one year customer service experience in a healthcare related field Self-starter who knows how to identify opportunities with recommended solutions Creative problem-solving skills Possess strong written, verbal, and presentation skills with diverse populations Ability to multi-task and stay calm and work as a team player in a fast paced environment while ensuring attention to detail Strong computer skills and typing skills If you meet these qualifications, and you would like to support building our amazing team, please Apply or contact us @ 541.687.8851 We look forward to meeting you!
