Job DescriptionJob Description:The Care Management Associate supports the RN Care Coordinator utilizing excellent organizational and communication skills and showing a personal commitment to providing compassionate care.Education Qualifications:Key Responsibilities:Provides communication with patients, care teams, and other stakeholders to ensure that all relevant information is available to provide the best possible quality of careAssists the RN Care Coordinator with scheduling appointments, coordinating care, providing follow-up services to patients, submits data to health plans, and any other duties as assignedTracks and maintains patient records and insurance informationProvides administrative support including data entry, documentation in the Electronic Medical Record, and other duties as requiredCollaborates with other departments to ensure the efficient delivery of care while demonstrating a strong commitment to providing compassionate and quality careRequired Qualifications:Medical Secretary, Health Unit Coordinator, or similar programMinimum of 2 years' experience in a medical setting if no education meets the Education Requirements listedJob Location: Ely ClinicShift Rotation: Day Rotation (United States of America)Shift Start/End: Day Shift/Day ShiftHours Per Pay Period: 56Compensation Range: $15.95 - $23.93 / hourUnion:FTE: 0.7Weekends:Call Obligations:Sign On Bonus:
