Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona.
CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
Care Manager The Care Manager is responsible for assisting individuals struggling with stimulant or opiate use disorder in need of recovery housing.
The Care Manager will help residents sustain recovery through service connection and support and ensures all residents admitted into the recovery program have appropriate access to resources, and program orientation.
The position is responsible for helping residents develop a recovery plan and connection to community resources (i.e., employment, benefits, identification, education) that will help sustain recovery and build stable lives in the DC community.
The Care Manager will review and update recovery plans routinely to ensure residents execute their recovery goals.
CBI employees must embrace the recovery philosophy and promote a safe recovery environment for clients.
Skills/Requirements: Develops 6-month recovery plans in partnership with residents to ensure appropriate connection to ancillary services.
Provides information on resources for employment, benefits, identification, and other resources in the community.
Assists, as needed, in conducting room checks and face-to-face visits with residents.
Reviews housing policies and orientation with residents.
Assists with group community and/or house meeting facilitation.
Works directly with Resident Monitor and Program Director to discuss issues or barriers residents may be experiencing.
Assists in the development of rules and policies for the recovery home setting.
Provides community education on the housing referral process.
Educates and assists residents with connection to other community service agencies/treatment providers and/or resources.
Ensures residents complete the Opioid Overdose Prevention & Naloxone Education (Community) training.
Assists with report deliverables as necessary including the GPRA and follow-up.
Documents all activities and communicates routinely with the Program Director.
Other duties as assigned by supervisor and/or manager and/or administrative support.
