Make a difference as a Care Manager Outreach Services

As a new Care Manager Outreach Services, you will be responsible for conducting client assessments, developing individualized care plans, and coordinating services to meet clients' needs.
You will collaborate with community partners, provide advocacy and support to clients, and maintain detailed documentation of all interactions.
Additionally, you will assist clients in navigating social service systems, ensuring they receive the assistance and resources they require.
