The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco.  The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services.  We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve with a focus on the Latino Spanish-Speaking Communities. The Mission Neighborhood Resource Center (MNRC) was founded in 2002 as a community response to the neighborhood's growing homeless population, the Mission Neighborhood Resource Center advocates for housing and resources to enhance the physical, social, emotional, and economic health of homeless residents in the 16th street corridor and surrounding Mission neighborhood. MNRC employs a service model based on harm-reduction principles and practices that focus on community building, basic human needs, and integrated health services for a diverse population as they move toward greater stability and well-being. The Mission SROC, a partnership of Dolores Street Community Services (DSCS), San Francisco Anti-Displacement Coalition, and Women's Community Clinic, employs a holistic approach to improve the living conditions and safety of low-income residents of Single Room Occupancy (SRO) Hotels in the Mission District. We are currently looking for qualified candidates for our Case Manager at Mission SRO Collaborative role. Please note: this role requires an applicant to be bilingual (English/Spanish). This position reports to the Director of Resource Center and is a Regular, Full Time, Non-Exempt role with a starting hourly rate range of $26.92 to $28.94 with full benefits. Primary Objective: In conjunction with the multidisciplinary treatment team, the Mission SRO Collaborative (MSROC) the Case Manager will provide outreach, referrals, case management services and crisis counseling to Mission SRO Collaborative tenants at Dolores Street Community Services (DSCS). The Mission SROC, a partnership of DSCS, Causa Justa/Just Cause, Mission Neighborhood Resource Center, employs a holistic approach to improve the living conditions and safety of low-income residents of Single Room Occupancy (SRO) Hotels in the Mission District. Essential Functions/Responsibilities: The Case Manager will assist tenants with stabilization support, including providing them with a warm hand off to MNRC's programs and San Francisco system-wide services as needed.Will conduct in-home outreach to SRO hotel in the Mission District and will act as MNRC's liaison to MSROC.Perform individual comprehensive assessments of Resource Center clients' psychosocial history, mental health status, medical needs, housing, food, and other needs for basic support. Determine eligibility and necessity for case management, medical, and behavioral health treatment, if appropriate.With clients' input assistance, develop a care plan delineating steps necessary to accomplish activities, goals, and objectives. Implement the plan through the provision of interventions, coordination with integrated services team and/or through facilitated referrals.Assist clients in compiling documentation, filling out forms for immigration, and public benefits' eligibility, including County Adult Assistance Programs, Social Security disability programs, and Medi-Cal, among others.Provide warm hand-off to connect tenants with MNRC programs and medical clinics and to access the San Francisco medical and behavioral health system programs as needed.Work closely with the Collaborative to respond to tenant needs, with a particular emphasis on intensive case management to assist with stabilization and eviction prevention.Participate in at least one daytime outreach shift per week, accompanied by a participant of the MSROC leadership program.Conduct SRO home visits and attend appointments with clients.Maintain SRO Tenant-spec-office hours. Develop and maintain knowledge of basic Tenants Rights/Housing Rights law in San Francisco.Create a hospitable environment and provide services to clients with the utmost dignity and respect.Utilize a participant-centered, harm reduction model of services.Maintain documentation for case management activities as required by funding sources.Regularly review files and monitor activities of clients with follow-up contact in-person or via phone.Attend relevant meetings, including staff meetings and staff training. Participate in weekly Mission SRO Collaborative coordination team meetings.Perform other duties as assigned. Qualifications: Education Bachelor's Degree in Social Work, Psychology, Counseling or a related field or at least two years paid work experience with homeless adults with multiple disabilities.ExperienceBilingual Spanish/English required.Past experience working with homeless adults and dually and triple-diagnosed individuals desired.Must be willing to provide support, advocacy, and develop relationships following harm reduction principles with a very diverse homeless population, including Spanish speaking immigrants, day laborers, women, LGBT, African Americans, active drug users, and people with mental illness and other disabilities.Ability to work with de-escalation techniques, conflict resolution, and emergency response.Candidates must have excellent verbal communication skillsMust have excellent charting, record-keeping skills and assist in the development of a community resource list/file.Must maintain professional, respectful, honest interactions with participants, staff, and outside agencies. Tools & EquipmentMust be proficient in MS Word, Excel, and Outlook.  PowerPoint experience desirable. Must be able to operate general office equipment, copier, printer, fax machine, and phone.Confidential & Sensitive InformationAbility to work professionally and ethically in a multi-cultural team setting, including the ability to maintain a high level of confidentiality.Candidates must have good working knowledge of and a sensitivity to individuals with substance abuse and/or mental health issues such as: PTSD, psychosis, borderline behaviors, bi-polar, etc.; working knowledge of crisis intervention techniques and ability to implement them; and strong knowledge of social services and mental health resources for low-income adults.  To learn more about our organization, please visit our website at www.mnhc.org.
