Summit Medical Consultants is a rapidly growing Physician Practice. Founded in 2015 with 3 employees, we have expanded to approximately 75 employees and are continuing to grow. If we were a hospital in Colorado, we would rank 9th by number of admissions per year. Our mission is to provide patients and their families with robust clinical services throughout their Acute Inpatient Rehab, Skilled Nursing, Long Term Care and Assisted and Independent Living stays. We coordinate with the Hospitalists and Specialist physicians and surgeons, Physical, Occupational, and Speech Therapies, Case Management and Psychiatry at the facility level to provide compassionate and coordinated care.
Job responsibilities include, but are not limited to:
Assessing patient's physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plansWorking with clients, family and friend support networks and health care professionals to put care plans in placeRecording clients' progress including charting referrals, home visits and other notable interactionsEvaluating clients' progress periodically with clinical provider team and making adjustments as needed to improve outcomesCoordinate discharge planning for skilled nursing patientsManage and build relationships with Home Health and Hospice partnersAssist Practice Management team in tracking, monitoring and intervening in ACO/ISNP and Optum patient metricsAssist provider teams in Annual Wellness Visits within our Long Term Care and Skilled nursing facilitiesTrack and implement improvement plans to decrease return to hospitalizations and improve overall health and wellness for our patientsCritical thinking and problem solving to determine the best plan of care after assessing patient's chart, reviewing with clinical providers and implementing interventionsManage and communicate barriers to Practice Management teamTransitional care management program- schedule, assist and complete TCM calls and VisitsAssist with TeleMedicine visits as assigned by Practice ManagerMaintain positive professional relationships with company vendorsBuild and maintain new relationships with facilities and Home health VendorsTraining care coordinatorsAdministrative duties as assignedOversight of care coordination teamDelegate appropriate assignments to care coordinator teams MA/LPNParticipate in admin on call rotation with other admin personnelManage patient caseload for entire practice with support from Care Coordinators and Triage Manager This description is intended to serve as an outline.
