JOB TITLE: Case Manager  Bargaining Unit Status:  Non-Bargaining Unit FULL-TIME ? , OR PART-TIME ?(hours per week: 40) Work Hours/Schedule: 7:00 AM-3:30 PM, M-F EDUCATIONAL QUALIFICATIONS:
Bachelor's degree from an accredited college in rehabilitation, counseling, psychology, sociology, social work, or a related discipline (a combination of equivalent education and work experience may be considered). EXPERIENCE QUALIFICATIONS: Three to five years' experience in a rehabilitation, non-profit facility or comparable setting in the area(s) of vocational rehabilitation, non-profit facility, substance abuse, mental health counseling, service delivery or case management. Proficiency with Microsoft Office suite and general computer operation is required. Working knowledge of basic case management principles. Strong working knowledge of disability management and awareness. Additional Preferred Qualifications:  Broad knowledge base of disabilities and issues influencing those with various conditions. Professional Certification of or equivalent certification preferred in:  N/A OTHER QUALIFICATIONS: Must be authorized for coverage under agency insurance if operating agency vehicle, have good driving record, valid Alabama driver's license, and/or required level of personal auto insurance. Must be able to satisfactorily complete a background check, per company requirements. Ability to communicate with tact and maintain confidentiality. Adequate ability in communicating in spoken conversation – adequate volume speech. Adequate ability in writing/reading/mathematics. Ability to effectively build and maintain positive working relationships with management and peers. Strong organizational, project planning, and management experience with proven analytical and multi-tasking skills. Superior listening, verbal, and written communication skills. Proficiency in general reporting and basic case file management. IMMEDIATE SUPERVISOR:
Program Coordinator JOB RESPONSIBILITIES SUMMARY:
Provide vocational services to individuals with disabilities ensuring they remain gainfully employed.  Must be timely and detail oriented in relation to case management requirements.  Maintain communication with referral agencies, family members, workplace supervisors and other involved parties.  Remain complaint with AbilityOne and CARF regulations and adhere to company policies related to service provision.  ESSENTIAL FUNCTIONS: Have working knowledge of current contract and referral source requirements as well as CARF standards, DOL, social security guidelines and Phoenix mission and internal policies. As necessary develop, write and present progress reports and discharge summaries documenting performed activities. Complete training as defined by the Phoenix Professional Development Plan. Represent Phoenix in the business community by involvement in civic and professional organizations. Provide information as requested for various statistical requirements. SPECIFIC RESPONSIBILITIES RELATED TO THIS POSITION MAY INCLUDE ANY OF THE FOLLOWING:
Case Manager: Partners with Industry and Supported Employment Provide employment readiness training (group and individual), assessment, job development, job placement and follow up for referred persons. Serve as a facility consultant in regard to employment readiness of program participants. Serve as a primary resource person for information on current employer needs. Participate in mission based community activities. Provide services in response to referral source needs and in regard to internal financial requirements in a timely manner. Case Manager: Vocational Evaluation Develop and implement, with program participants, individualized plans of evaluation services leading towards his/her desired program outcome and coordinate with the referral source. To administer and score vocational testing and interpret results to program participants, families and referral sources, as appropriate. To make determinations with the program participant in regard to: least restrictive work environment, readiness for work, strengths and limitations as they relate to employment and supports required to secure and maintain employment. To develop and write progress reports which document program participant progress toward goals.  To develop and write a comprehensive final evaluation report to referral source. To have working knowledge of DOL (and other) vocational/career and life planning resources.  To be familiar with jobs in the local labor market and job analysis processes. To make appropriate referrals both in and outside of the center for additional services, such as vocational training, counseling and medical management. To provide input to the other members of the team about departmental and overall policies and procedures. To provide diagnostic testing in school transitional programs through mobile or in-house services. Case Manager: Employee Support Services Possess or ability to gain a governmental secret clearance and pass a background check within a reasonable time period as determined by employer. Ability to gain unescorted access to Redstone Arsenal. Provide case management services to Phoenix employees certified as having a disability to include: direct intervention, counseling session and mediation sessions; development and maintenance of competitive employment evaluations and reasonable accommodations per AbilityOne guidelines; develop and present disability awareness and sensitivity trainings; improvement plan generation and reporting and file maintenance. Maintain contact and communication with appropriate supervisors, project managers and other Phoenix Vocational staff to provide effective case management services. Case Manager: Ability One Employment Possess or ability to gain a governmental secret clearance and pass a background check within a reasonable time period as determined by employer. Ability to gain unescorted access to Redstone Arsenal. Build referral relationships with agencies in the surrounding geographical location in order to fill Phoenix AbilityOne job openings. Develop systems and processes in order to identify qualified individuals, primarily those with disabilities who have a desire and need for employment with Phoenix. Work closely with Phoenix's HR department for coordination of services. Maintain contact and communication with appropriate supervisors, project managers and other Phoenix Vocational staff to provide effective case management services. Provide case management services to referred individuals. WORKING CONDITIONS/PHYSICAL REQUIREMENTS: Adequate hearing required for conversation, telephone use, and emergency information systems. Ability to be independently mobile on all surfaces and situations. Ability to move safely about work area. Ability to retrieve items from high and low areas. Ability to perceive size, discrimination, temperature, shapes, and/or textures. Physical stamina to work at least 8 hours per day. EMOTIONAL CAPACITIES: Must be able to change routine in a positive manner. Must be able to follow through on assignments independently. Must have good organizational skills. Must have a long attention span in order to listen to people, perceive the real problems and bring issues to a successful conclusion. Must be able to analyze many variables and choose the most effective course of action for the organization at any given point in time. Must possess effective oral and written communication and interpersonal skills. Must be able to maintain confidentiality. Must have stable work behaviors daily. Must have adequate individual coping skills. Must be able to maintain good work attendance. Must be able to resolve problems, handle conflict, and make effective decision even in emergency situations. Must be able to exercise good judgment to maintain a safe work environment.
