Job Description:
Join a world-class academic healthcare system, UChicago Medicine, as the Nurse Navigator to build and lead a new program with Chicagoland Childrens Health Alliance (CCHA).
We are seeking a results-oriented and compassionate nurse who will act as a clinical liaison for pediatric patients and their families across multiple medical services, clinical departments and facilities.
Who you are: Registered Nurse (RN) with a current Illinois license.
Minimum of 2 years of experience in a nurse navigator, care coordination or case management role required.
Clinical experience in pediatric surgery strongly preferred.
Must be eligible to work with Advocate, Northshore, and UCM health information systems.
Passionate about providing an exceptional experience to pediatric patients and their families.
Critical thinker who can problem solve and troubleshoot across multiple health organizations.
Excellent communication and interpersonal skills needed to work effectively with patients, family members, and physicians.
What youll gain as CCHA Nurse Navigator: Opportunity to make an impact in a newly created role that will allow for integration between 3 locations seamlessly with the mission of providing an excellent patient experience through challenging health situations.
What youll do as the CCHA Nurse Navigator: Acts as liaison for pediatric patients across the CCHA network.
Serves as a clinical partner to facilitate a seamless patient experience across the CCHA network.
Acts with urgency and escalates barriers to access for patients to ensure patient appointments are within 7 days.
Coordinates prior authorization and release of records preceding initial visit in communication with program providers.
Coordinates joint appointments and referrals prior to first visit in communication with program liaison and providers.
Oversees the scheduling of new patients initial appointment with the appropriate provider.
Develops and coordinates development of system education materials, clinical pathways and quality studies.
Serves as call center contact and escalation point for intake coordinators.
Coordinates appointments and facilitates follow-up testing.
Must comply with UCMCs COVID-19 Vaccination requirement as a condition of employment.
