Overview
LifeLong Medical Care has an exciting and rewarding opportunity searching for an On-Call/Per Diem Certified Nurse Midwife (CNM) to join the team at our Brookside San Pablo Health Center.
The CNM provides general prenatal, postpartum, and gynecological health services in a community clinic setting.
This position is represented by UAPD.
Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of UAPD, as defined in the CBA. We provide medical, dental, behavioral health, and school-based services at more than 15 primary care sites across Oakland, Berkeley, and West Contra Costa County.

Benefits: We offer a competitive annual salary of $123,000-$146,000, depending on years since licensed, and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs.
LifeLong is qualified for federal and state loan repayment programs (HPSA score is 21). Acceptance rates may vary year to year based on funding levels and the number of applicants.

Responsibilities
Provides prenatal, postpartum, and gynecological health care including: screening, health maintenance and disease prevention, management of acute and chronic gynecological conditions, and making appropriate referrals and follow-up for diagnostic and specialty services.
Coordinates care with other health team members including behavioral health staff.
Acts as an advocate for the patient and liaison with outside agencies as appropriate.
Provides verbal and written education and counseling to patients, families, and caregivers.
Maintains patient records necessary for clinic reporting, billing, and quality improvement.
Encouraged to provide some care as shared medical (group) visits Consults with LifeLong physicians as appropriate.
Participates as an active member of LifeLong Clinicians Association and abides by its Code of Conduct.
Attends clinical team meetings.
Participates in quality performance activities and improvement initiatives.
May be involved in the supervision and teaching of residents, students or fellows.
Tuesday evenings greatly preferred as part of schedule.
Qualifications
Commitment and demonstrated ability to care for a variety of people from underserved populations, with diverse educational, lifestyle, ethnic and cultural origins.
Positive approach to caring for patients in partnership with colleagues.
Ability to take initiative, work independently under minimum supervision and make sound judgments within established guidelines.
Ability to communicate effectively and work with individuals from a wide range of ages, abilities, and backgrounds.
Ability to work in a team environment with a number of licensed and non-licensed staff with different work styles and support needs.
Education and Experience Current California CNM license, including furnishing license.
Minimum of one year experience delivering perinatal and gynecological care to people across the lifespan.
Community clinic experience preferred.
Experience as part of a team-based care model preferred.
Knowledge of Contra Costa County resources is desirable.
Bilingual - English/Spanish is desirable.
Experience with electronic health records, specifically eCW and Epic, strongly preferred.
Experience working with language translation services is desirable.
