Elwyn is currently seeking a
Certified Nursing Assistant
(CNA). The role focuses on providing essential care and support to patients within the comfort of their own homes. This position will work closely with patients and their families to ensure their well-being and maintain their quality of life. Responsibilities include assisting with daily activities, monitoring health conditions, and promoting a safe and nurturing environment.
Work Schedule : 1st, 2nd & overnights available
Pay Range : $21.00 p/hr., $2,000 Sign on Bonus
DUTIES AND RESPONSIBILITIES:
Assist patients with bathing, dressing, and grooming
Help maintain personal hygiene and comfort
Monitor prescribed medications according to the care plan
Monitor medication schedules and document any changes
Document assessment and align goals with the Person-Centered Treatment Plan within the EHR (Electronic Health Record) system
Adhere to HIPAA regulations and maintain patient confidentiality
Assist patients with transferring in and out of bed or chairs
Ensure safe movement within the home
Prepare meals based on dietary requirements
Encourage proper nutrition and hydration
Regularly check patients' vital signs (such as blood pressure, pulse, and temperature)
Report any significant changes promptly
Engage in conversations and provide emotional comfort
Foster a positive and supportive environment
Perform cardiopulmonary resuscitation (CPR), crisis intervention and medication administration using agency-trained protocols
Maintain current state-mandated training and certifications
Housekeeping duties, including laundry, tidying up, and organizing
Ensure a clean and safe living space
May work evening and weekend hours according to the team model and an on-call rotation will be required as necessary
QUALIFICATIONS, EDUCATION AND EXPERIENCE:
High school diploma or GED required
Completion of a certified nursing assistant training program
A valid CNA license certification required
At least one year of experience as a certified nursing assistant preferred
Ability to adapt to various home environments
Effective communication with patients and their families
Non-profit human service experience highly preferred
Demonstrated ability to work effectively as part of a team
Ability to work in a fast-paced environment and manage competing priorities
Demonstrated strong attention to detail
Demonstrated strong time management and organizational skills
Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
Must be able to perform CPR and crisis intervention using agency-trained protocols
Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication
Demonstrated (basic, intermediate, advanced) experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint; Access, Publisher and report-writer experience preferred
Experience using and knowledge of Electronic Health Record system
Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
PHYSICAL DEMANDS/ENVIRONMENTAL PROFILE:
Ongoing interaction with consumers and staff, potential exposure to communicable diseases, blood-borne pathogens, medicinal preparations and other common conditions in a healthcare environment, and interaction with consumers who may display aggressive and/or self-injurious behaviors.
