Responsibilities
• Perform personal care needs and attend to patient requests• Report to the Case Manager if unable to perform a certain task• Provide positive communication techniques to patients and families• Ensure patient safety and properly use equipment• Provide personal care including baths, hygiene, and changing bed linen• Assist with dressing, undressing, and feeding patients• Plan and prepare meals as assigned• Take and record vital signs• Prevent skin breakdown of patients• Report patient condition changes to Case Manager• Assist with ambulation, exercise, and transferring• Assist with self-administered medications and offer bedpans/urinals• Assist with light laundry needs• Perform range of motion and other simple procedures• Provide respite care when on-site• Keep patient's living area clean and orderly• Adhere to documentation and care procedures• Participate in quality assessment and improvement activities• Respect patient's environment and personal needs• Meet state training requirements and have minimum experience in providing personal care• Ability to read and follow instructions, understand hospice philosophy• Satisfactory references from previous employers• Ability to work independently with little supervision• Comfortable with issues of death/dying• Maintain current CPR certification• Demonstrate tact, patience, and good personal hygiene• Successfully complete probationary period• Strong oral and written communication skills• Good organizational skills
Requirements
• Minimum of 75 hours of classroom and supervised practical training• Minimum of 6 months experience providing personal care• At least 18 years of age• Ability to read and follow written instructions• Familiarity with hospice philosophy• Satisfactory references• Ability to work with little direct supervision• Comfortable with issues of death/dying• Current CPR certification• Tact, patience, and good personal hygiene• Successful completion of probationary period
