The Client Wellness Manager (LPN) will oversee the daily operations of the in-home care services, including managing a team of caregivers, coordinating care plans, and ensuring compliance with healthcare regulations.
This individual will serve as the key point of contact for clients and their families, ensuring that our services meet the needs of each client while maintaining high levels of satisfaction and care quality.
Key Responsibilities: Develop, implement, and oversee personalized care plans for senior clients based on their needs and healthcare requirements. Supervise and manage a team of caregivers, ensuring they follow care plans and provide high-quality care. Conduct regular assessments of clients' health status and adjust care plans as necessary in collaboration with healthcare providers. Coordinate and communicate effectively with clients, families, and healthcare professionals to provide updates on client care and address any concerns or issues. Ensure compliance with state and federal healthcare regulations, as well as company policies and procedures. Conduct training and development sessions for caregivers, ensuring they are equipped with the skills needed to provide excellent care. Participate in the recruitment and on-boarding of new caregivers, including conducting interviews and background checks. Schedule client visits and caregiver shifts, ensuring all client care needs are met in a timely manner. Provide hands-on care as needed, particularly for clients with more complex medical needs. Maintain accurate client records using electronic medical records (EMR) systems and ensure confidentiality of all medical and personal information. Act as a resource for caregivers, providing guidance and support for challenging client cases. Monitor the quality of care provided, conducting client satisfaction surveys and making necessary improvements to enhance service delivery. Qualifications: Current Licensed Practical Nurse (LPN) license. Minimum of 2-3 years of experience in a clinical or home care setting, with experience in case management or client care coordination preferred. Previous experience managing a team of caregivers or healthcare professionals is a plus. Knowledge of senior care services, including personal care, dementia care, and chronic illness management. Strong understanding of state and federal healthcare regulations, particularly those related to home care and eldercare. Proficiency in using electronic medical records (EMR) and scheduling software. Excellent communication and interpersonal skills with the ability to build rapport with clients, families, and healthcare providers.
