Summary
ABOUT THE ROLE:
Responsible for and assists with assessment, planning, implementation and evaluation of patient care in a professional, confidential and caring manner. An active member of a Medical Home Care team with physicians and advance practice practitioners to provide safe, accurate and quality patient care, which includes a focus on chronic disease management, health maintenance and acute conditions. Ensures that a follow up plan of care and educational needs are met and coordinates the flow of patients, phone calls, lab work, and nurse visits to facilitate quality patient care and customer service goals. The majority of the time will be spent in the Triage/Call Center.

Collects subjective and objective data and contributes to the assessment of the patient and documents appropriately in the patient chart for both in person visit or via telephone
Makes accurate and timely observation and documents changes in the patient's condition in the patient chart accurately and according to policy.
Performs a pre-visit chart review for completeness including areas of health maintenance exams, medications, tests and visits. Assists in the coordination of these tasks per provider protocols.

Performs a huddle with provider regarding identified questions, individualized patient care and wellness needs.
Performs basic clinic nursing skills.
Schedules referrals such as radiology, lab testing, or referral visits and obtains prior authorizations from insurance companies as required for patient care.
Works efficiently and closely with other departments for the care and treatment of the patient.
Assists in organizing, prioritizing and coordination of care. Identifies teaching and learning needs and communicates with the provider, patient, family and others. Involves other resources and referrals that may be needed as directed by the provider.
Keeps flow of patients in accordance with time of appointment in order to avoid delays for the provider and undue waiting for the patient.
Administers medication according to policy and provider order.
Assists the provider with examinations and procedures.
Explains tests and other procedures as needed for adequate patient education.
Chart preparation is thorough, complete, and done prior to the start of each office day.
Assists in the completion of medication refill process per provider approved protocol.
Assists in the Coumadin clinic per provider protocol.
Utilizes positive phone etiquette skills when taking and returning phone calls from patients and communicates accurately with appropriate staff in order to provide timely response to the patient.
Maintains a neat, organized, and safe environment for the patients, visitors, and CAMC team.
Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
Participates in meetings, committees and department projects as assigned.
YOUR EXPERIENCE INCLUDES (PROVEN PERFORMANCE IN):
Graduating from a professional nursing program – required
NE Registered Nurse or Licensed Practical Nurse licensure – required
Certification in Basic Life Support (BLS) – required
