POSITION SUMMARY
Clinical Care Coordinators function as liaisons between patients, providers, and the healthcare system.
Clinical Care Coordinators ensures that patient needs, discharge planning, and care coordination efforts are all coherent with care management criteria.
Clinical Care Coordinators must remain cognizant of patient necessity, levels of care, medical conditions, discharge plans, and medications.
Provides assistance to physicians with patient related case management and quality metrics.
Duties may also include patient instruction, care orientation, and coaching.
ESSENTIAL DUTIES AND RESPONSIBILITIES Care Coordination: Assists patients in performing office procedures and duties related to patient care as requested adhering to documentation standards based on Office Protocols.
Acquires thorough patient histories and documents preventative care, assists with medication reconciliation and transcribes family history and any forms based on Office Protocols.
Performs basic Medical Assistant functions based on assigned office such as Injections, Immunization, Retinal Eye Exams, HbA1C, Urine Microalbumin, and any pending quality metrics.
Reviews each referral and laboratory requisition for correct patient, accurate tests and diagnosis and the correct laboratory.
Obtains waivers and consents and documents thoroughly what procedures were performed and instruction given to patient using electronic medical record or following guidelines in Office Protocol.
Appropriately prepares examination rooms immediately after patient's visits, as needed.
Exhibits tact, courtesy, respect, cooperation and support in every aspect of their job when with patients, staff and providers always adhering to.
Perform duties with speed and accuracy while observing the guidelines set forth by OSHA, HIPPA, DHS, CLIA, EPA and specific contracts.
Assists patients through the healthcare system by operating as a patient advocate and health systems navigator.
Coordinates continuity of patient care with external healthcare organizations and facilities.
Coordinates continuity of patient care with patients and families/caregivers following hospital admission, discharge, and Emergency Department visits.
Reports care barriers and challenges to physician or designee Conducts comprehensive, preventive screenings for patients and/or assists disease management support staff in daily patient interactions as needed.
Supports care managers to ensure Health Risk Assessments and Health Action Plans are conducted for patients enrolled in Disease Management.
Crossed trained to participate in the implementation of Enhanced Care Management (ECM) with the goal of ensuring that eligible Medi-Cal beneficiaries receive enhanced care management and coordination as needed.
Supports patient self-management of disease processes and promotes behavioral modifications self-intervention.
Promotes clear communication amongst interdisciplinary care team members by ensuring awareness regarding patientcare plans.
Facilitates patient medication management based upon standing orders and protocols.
Participates as a successful team associate supporting data collection, health outcomes reporting, clinical audits, and pragmatic evaluation.
Participates in the evaluation of clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
Under the direction of the Manager of Population Health and Health Programs Supervisor determine which projects will take priority at any time for the Martin Luther King Jr. Community Medical Group.
Works collaboratively with other clinical personnel assigned to the same patient.
Assists care managers in the care of patients with chronic diseases following established protocols and systems for disease management in collaboration with providers.
Coordinates project plans including project timelines Provide direction and support to the project team as required Tract project deliverables using appropriate tools Identify project risks and recommend appropriate resolutions Projects defined, Physician Dashboards and Quality Metrics Assists in creation and submittal of Medical Group invoices Other duties as assigned.
POSITION REQUIREMENTS A.
Education Medical Assistant Diploma/Certificate from accredited school CMA Certification Preferred Maintaining certification by completing CME's and keeping certifications up to date.
High School graduate or GED.
BLS certified and knowledge of emergency procedures B.
Qualifications/Experience Minimum of one year experience in a medical practice environment.
6 months of project coordination experience preferred Healthcare and/or Hospital experience preferred A team player that can follow a system and protocol to achieve a common goal Highly organized and well developed oral and written communication skills Confidence to communicate and outreach to other community health care organizations and personnel Demonstrates sound judgment, decision making and problem-solving skills C.
Special Skills/Knowledge Proficient to expert computer skills utilizing Microsoft Office especially Word and Excel Critical thinking Resourcefulness Leadership Knowledge of healthcare delivery systems Bi-lingual Spanish helpful but not required Knowledge of medical technology, procedural terminology and diagnostic codes.
Ability to maintain a professional and patient care-oriented attitude.
