**APPLICANT MUST HAVE A VALID, UNRESTRICTED INSURABLE DRIVER'S LICENSE** **RESUME AND REFERENCES ARE REQUIRED** ESSENTIAL DUTIES, FUNCTIONS AND RESPONSIBILITIES:Develops and implements comprehensive care plans with patient, family, and team, evaluating and updating these plans based on needs.Coordinates with family, Ambulatory Clinics, and other services; facilitates meetings and attends community meetings as necessary.Cultivates and supports assigned care & subspecialty co-management with timely communication and follow-up.Educates, counsels, and supports patients; intervenes or facilitates referrals in crises.Builds care relationships and supports the primary care-giving role of the family.Assists with identifying patients with chronic care needs and maintains a chronic disease registry.Understands eligibility requirements and referral processes for public and private sector services.Serves as the point-of-contact and advocate for family and community partners.Consults and provides resources to FDIHB staff regarding patient care.Develops and maintains policies related to care coordination of complex patients.Serves as a quality improvement team member; helps measure quality and implement practice improvements.Maintains professional growth and development through continued education.Attends to clinical needs of complex patients in the clinic and inpatient unit.Provides skilled and comprehensive nursing care as needed.Offers outreach and education to the community regarding chronic care issues.Develops educational materials and provides instruction to meet audience needs.Fosters educational development using appropriate theories and principles.Fields queries from the community regarding chronic care issues.Completes all required training by the indicated due date.Promotes compliance with established standards.Performs other duties as assigned.MANDATORY MINIMUM QUALIFICATIONS:Experience: Two (2) years of Registered Nurse experience in a clinical, medical/surgical, acute and/or critical care setting.
Education: Associate's of Science degree in Nursing from an accredited nursing program.
License: Valid and unrestricted Registered Nurse license in any U.S. State or Territory.
Certificate: Valid American Heart Association certification in Basic Life Support (BLS).
