Program Description Felton's Older Adult Service Team (OAST), based in Alameda County, offers specialized, multidisciplinary geriatric mental health services and other aging-related social services to our diverse community of elders.
Our comprehensive mental health services include clinical case management, medication management, individual and group therapy, community integration, and specialized outreach.
The clinical team includes a psychiatric care provider, clinical case managers, case managers, and peer/community specialists.
We serve people ages 60 and over living with severe mental illness throughout Alameda County; those established with the Alameda County system of care and those who are newly connected with services.

Job Description This position is part of a multidisciplinary team serving older adults, aged 60 and over, with severe mental illness (SMI) and co-occurring disorders.
Under the supervision of the OAST Program Director, the Clinical Case Manager provides comprehensive case management services, utilizing their clinical knowledge and experience.
This includes assessing and identifying the clients' needs, working to connect them with the appropriate services- medical, financial, substance abuse treatment, housing, transportation, etc., and advocating on their behalf.
Working collaboratively with the client, the Clinical Case Manager evaluates the needs of clients and develops a treatment plan which tailors services to each individual client.
The Clinical Case Manager provides individual and group therapy, and crisis intervention as appropriate, and is responsible for maintaining accurate and detailed documentation.
Familiarity with MediCal billing is helpful.
OAST employs a client-centered approach to provide culturally competent and recovery-oriented services to a diverse population of seniors living in Alameda County.
Referrals for the OAST caseload come from Alameda County Behavioral Health ACCESS.
Once stabilized, clients are stepped down to a less intensive level of care according to their needs.

Specific Responsibilities Works closely with an interdisciplinary team of clinical case managers, psychiatric physician assistant, psychiatrist, and community/peer specialists to provide intensive services to older adults with complex behavioral health and medical needs.
Performs intakes for older adults with a severe mental illness in a Felton clinic, in the client's home, or in an appropriate field-based location.
Provides case management services, including evaluations, treatment plan development, assisting clients in accessing housing, financial, medical and other needed services, and advocacy.
Provides therapy and other mental health interventions.
Coordinates closely with other service providers and clients' natural supports.
Provides crisis intervention and management to prevent hospitalization.
Maintains clinical records after each client contact or service, in accordance with mandated standards of Felton Institute, Alameda County, and MediCal.
Participates in team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.
Commits to ongoing professional development in the geriatric specialization of mental health services by participating in trainings offered by Felton Institute or outside providers.
Provides information regarding community resources to the general public as needed.
Other duties and responsibilities that may be assigned as needed.
Minimum Qualification A minimum of two years of case management and mental health treatment for at-risk older adults, including co-occurring disorders.
Masters degree or higher in Social Work, Counseling, or Psychology and active registration with the BBS - AMFT, APCC, ASW, LMFT, LPCC, LCSW, highly preferred.
May consider an exceptionally experienced BSW or other bachelors degree with 5 years case management experience.
Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention.
Ability to work cooperatively on a multi-disciplinary team and community partners and collaborators.
Knowledge of DSM V-TR, and experience with Medi-Cal billing requirements Valid CA Driver's License and vehicle.
Strong organizational skills Strong communication skills both verbal and written.
Strong interpersonal, organizational, and time management skills Ability to prioritize projects and follow through.

FLSA Status: Exempt
Salary Range: $93,000-$105,000 Annually (DOE)
