The PHP Clinical Educator has a passion for educating other health care professionals who work in a variety of healthcare settings that can include, but are not limited to, skilled nursing facilities, assisted living, and independent living communities, as well as via telehealth.JOB SUMMARYThe Personalized Health Partners (PHP) Clinical Educator is responsible for educating other healthcare professionals who work in healthcare environments with the knowledge and skills needed to provide the best possible care.

The clinical educator may develop and implement a method to evaluate the learning of peers as they provide care to patients.PRIMARY RESPONSIBILITIES AND ESSENTIAL FUNCTIONSDevelop a strong foundational education program for PHP.Provide ongoing performance feedback to clinical leadership throughout the year.Identify educational needs and work to develop and implement training.Develop and implement educational strategies for clinical team.Review clinical opportunities and recommend strategic plans to improve them.Develop relationships with the clinical leadership team and quality/coding team.Support strategic initiatives regarding value-based care.Practice ethically and in accordance with the Scope and Standards of Practice of their profession and Board Certification.Follow all state and federal regulations, guidelines, and laws.Additional duties of clinical educatorsCollaborate with the PHP leadership team.Be flexible to travel, on an as needed basis, to ensure that clinical professionals are trained to meet resident care and facility needs.Work in a remote-based position (at least 40% in the field, 60% administrative).Work closely with clinical leadership to provide performance reviews related to uncompleted Relias trainings, documentation audits, and quality/coding reviews.Work closely with the coding team to identify trends across the practice that warrant further education.Assist the clinical leadership team with developing, implementing, and training with ongoing educational needs.EDUCATION and QUALIFICATION:Required:Masters of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduateNational certification of AANP or ANCCActive, unrestricted state license/sAbility to obtain licensure in additional states in service area as requested (telehealth providers required to be licensed in OH, IN, WV, MD, VA, PA, MO)Active DEA licenseUS work authorization2+ years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult, or family practice settingAbility to work in a flexible environment and schedulePassionate about providing healthcare to the senior and low-income populationsPreferred Qualifications: 1+ years clinical educator experience as a Nurse or Nurse Practitioner in geriatric, adult or family practice settingTraining or experience in geriatric, assisted living, and/or skilled nursing settingExperience with coding, ICD-10 and HCC (Hierarchical Condition Categories)Experience or training working in a metrics, results driven healthcare modelBe open to professional development through training, obtaining certifications if necessary, and attending team meetingsExperience working in value-based care modelsExperience working in a collaborative healthcare setting to drive positive outcomes and achieve goalsTraining or experience in outpatient primary care settingJOB SKILLSMust have strong oral and written communication skillsMust be detail oriented with an ability to work well both independently and in a team settingExhibit a customer service approach with teams and residentsStrong time management skills required.
