The Hospitalist Clinical Navigator serves as a clinical resource in the office setting, communicating with Hospitalists and community physicians. They ensure continuity of care between acute settings and outpatient facilities by participating in triage phone calls, linking patients and families with available programs to help navigate the healthcare system, and facilitating all necessary forms and documents needed for patient satisfaction.
The Clinical Navigator comes with an Associate Degree in Nursing and a minimum of one-year experience as an office nurse in a family or internal medicine practice. They hold a current RN licensure in the state of Florida, and possess both a Basic Life Support (BLS) certification and an Advanced Life Support (ALS) certification from the American Red Cross or American Heart Association. Due to their proficiency in Microsoft Excel, Word, Outlook and Windows, they will assist in data tracking and trending and participate in Hospitalist Quality projects, among other duties.
Essential Duties and Responsibilities:

Provide indirect care to hospitalist patients and families
Assist in development and maintenance of appropriate safe staffing levels
Communicate with primary care physician regarding patient admission and discharge
Participate in triage phone calls, providing essential links and resources
Assist in coordination of new services and participate in Hospitalist staff development
