POSITION SUMMARY

The Clinical Pharmacist provides comprehensive pharmaceutical care through integrated drug distribution, patient counseling and teaching.
Clinical Pharmacists fill orders for medications, monitor patients drug therapies and provide drug information.
Clinical Pharmacist supervises and directs supportive staff.
In the absence of the Director of Pharmacy, a Clinical Pharmacist may be required to assume the essential responsibilities and perform the duties of the Director of PharmacyIn addition, the Clinical Pharmacist provides pharmaceutical care services to include advising physicians on matters pertaining to drug usage and control within assigned hospital locations; participate in the development and delivery of continuing education programs for Pharmacy, Patient Care Services, Medical staff and students.


Transplant Pharmacist Performs all Transplant Pharmacist duties Monday - Friday 8:00 AM to 4:30 PM (Inpatient Pharmacy to support transplant after clinic hours and on weekends) Provides comprehensive medication education to transplant recipients and families Collaborates with members of the transplant team in the care of transplant candidates and recipients Perform medication review for hospital discharge and post-transplant patients Provide medication counseling and discharge medication teaching for immunosuppressant therapy (for patients, family and caregivers) prior to patient's discharge Teach patients to fill pill boxes and provide additional assistance for those with needs Assist with prior authorization as needed Provide medication schedule to increase patient adherence Answer drug information queries for providers, nurses and patients Participates in Quality Improvement Projects and research studies that are related to pharmaceutical education as directed (no more than two non-pharmaceutical studies) Provide educational in-services for pharmacist and nursing staff Provide Journal Clubs/Topic Discussions for pharmacy students Supervise pharmacy technicians, residents and students as directed Participate in and/or coordinate any associated research projects for the transplant clinic patients as needed Pre-transplant Reviews medications for transplant recipients presented at patient selection meetings Evaluate medication profile for potential drug interactions Communicates with transplant coordinators to ensure medication interaction alerts are added to OTTR Enters Alerts in OTTR about patients on blood thinners Perioperative phase Evaluates, identifies, and solves medication-related problems for transplant recipients Review induction and maintenance medication protocols for immunosuppression Provides education to the transplant team on transplant medications and adherence to the medication The transplant team then provides this information to the transplant recipients and their family members Prepares and assists with discharge planning for transplant recipients Attend inpatient multidisciplinary rounds and document medication-related interventions in patient's EMR Make recommendations for infection prophylaxis during inpatient rounds and in the outpatient clinic Documents discharge plan for medications in patient's EMR Complete two notes prior to discharge for new transplant patients Communicates any medication concerns to post transplant clinic coordinators Evaluates transplant recipient medication regimens as indicated Communicates all transplant recipient medication issues and concerns to appropriate members of the transplant team Attends weekly patient selection meetings Attends monthly QAPI meetings Post operative Phase Counsel patients in outpatient clinic and review medication therapy plans Perform medication reconciliation and assist with dose adjustment for target immunosuppressive agents to therapeutic goal Assist with coordinating medication prescriptions and determine pharmacy cost to patient Monitor for duplication of therapy, GI prophylaxis and medication adherence Alerts transplant team of any potential drug interactions with immunosuppressive routine post-transplant Make recommendations regarding blood pressure medications, insulin management, acute pain and other chronic conditions Assist with coordinating medication prescriptions during discharge Evaluates compliance/adherence in post-transplant recipients as needed Living Donors Reviews any reported medication concerns about potential living donors with living donor coordinator Participates in living donor selection meetings Documents any concerns about living donor medications during selection meetings Peri Operative Documents discharge medications in living donor EMR Communicates any concerns for discharge medications to living donor team members Qualifications Education/Qualification Bachelor s Degree in Pharmacy 2 - 3 years of relevant experience Doctor of Pharmacy Degree (Pharm.D.)
preferred Skills Effective communication skills, both verbal and written Effective interpersonal skills Ability to maintain confidentiality Ability to adapt to multiple and changing priorities Knowledgeable of and complies with laws, rules and regulations that govern the practice of pharmacy.
Pyxis experience with knowledge of Pyxis Reports (i.e., optimization, load/unload, nursing narcotic access, etc.)
Some travel, to follow-up with Corporate, Financial, GPO, Compliance and Pyxis issues.
