Clinical - Senior Ltss Service Care Manager (Rn)

Location: This is a member facing role, will be making home or facility visits to members. Requesting candidates from Abilene, San Angelo area.

SHIFT:8 am to 5 pm, Overtime possible but not required during training

Duration: 6+ months

Walk me through the day to day responsibilities of this the role and a description of the project

SC will have 3 days of visits and 2 days of documentation weekly

SC will have a caseload and work assigned tasks (work items)

SC will complete STAR Kids Screening and Assessment Instrument, Individual Service Plan

SC will complete required service forms for each assessment

SC will conduct follow up calls and complete contractual telephonic outreaches

Expectations are to complete required assessments and documentation within mandated turn around time.

Job Description: Position Purpose:

Performs care management duties to assess and coordinate all aspects of medical and supporting services across the continuum of care for complex/high acuity populations with primary medical/physical health needs to promote quality, cost effective care. Develops a personalized care plan / service plan for long-term care members, addresses issues, and educates members and their families/caregivers on services and benefit options available to receive appropriate high-quality care.

Education/Experience:

Requires Graduate from an Accredited School or Nursing or a Bachelor's degree and 4 6 years of related experience

Bachelor's degree in Nursing preferred

License/Certification:

RN - Registered Nurse - State Licensure and/or Compact State Licensure required or

NP - Nurse Practitioner - Current State's Nurse Licensure required

Internal/External Groups with which the Candidate will interface:
Healthcare providers, internal departments

Required Skills/Experience:
NICU, Home Health, Pediatrics
Preferred Skills/ Experience:
Home Health, Pediatrics experience preferred.
